Guide For New Patients

If you are interested in becoming a patient, please request an appointment or call our office at (773) 296-670
In order to better serve you, here’s a quick checklist of items to prepare before your first visit:
- Complete your new patient paperwork prior to your scheduled appointment.
- We accept BlueCross BlueShield PPO and Blue Choice plans; please check your coverage with your insurance company prior to your visit.
- If you have a different insurance provider, we can provide you with a detailed superbill to submit out-of-network. Your insurance company may reimburse you for part of your visit.
- If you do not have insurance or are coming in for a self-pay service, please bring payment in the form of Visa, Mastercard, Discover, American Express, CareCredit, check, or cash. Payment is due at the time service is rendered.
