Perhaps thinking back on your own colonoscopy, it\u2019s reasonable to ask, \u201cDo I really need a cognoscopy?\u201d<\/p>\n\n\n\n<p>There are three groups of people I recommend give these tests serious consideration. (And at least a cognoscopy doesn\u2019t require a laxative.)<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li><strong>If mid- or late-life dementia runs in your family<\/strong>. (\u201cGrandma was showing confusion in her late 60s and spent her final ten years in a nursing home.\u201d)<\/li><li><strong>You\u2019re middle aged and experiencing too many senior moments.<\/strong> No one else notices, but you do, which is why it\u2019s called subjective cognitive impairment.<\/li><li><strong>You\u2019re middle aged and people around you have expressed concern about changes in your cognitive skills<\/strong> (missed appointments, late for meetings, third iPhone replacement). This is mild cognitive impairment.<\/li><\/ul>\n\n\n\n<p>Although it\u2019s been long established that a protein called amyloid deposited in your brain is the culprit behind the dementia that characterizes Alzheimer\u2019s, there are three primary amyloid triggers: inflammation, atrophy, and toxins.<\/p>\n\n\n\n<p>Before you do any detailed testing, ask your doctor to check your APOe (the Alzheimer gene) status. While dementia can occur among APOe-negative patients, being APOe-positive can increase your risk dramatically.<\/p>\n\n\n\n<p><strong>Here are tests for inflammation<br><\/strong>Click on the links to learn more.<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li><a href=\"https:\/\/www.mayomedicallaboratories.com\/test-catalog\/Clinical+and+Interpretive\/82047\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">hsCRP<\/a> (high sensitivity C-reactive protein) measures inflammation throughout your body.<\/li><li>A\/G ratio (ratio of albumin to globulin), a test that complements the hsCRP.<\/li><li><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/23451843\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">Ratio of omega 6 to omega 3 fatty acids in your blood cells<\/a> (ditto).<\/li><li><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/22026253\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">IL-6<\/a> (interleukin 6) and <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5436834\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">TNF<\/a> (tumor necrosis factor), both of which increase during chronic inflammation.<\/li><li><a href=\"https:\/\/www.alzdiscovery.org\/cognitive-vitality\/blog\/going-gluten-free-for-your-brain\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">Gluten sensitivity<\/a> (though simply eliminating gluten for 14 days and finding you feel better and then re-introducing it and feeling worse is an excellent self-test).<\/li><li><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/28372330\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">Leaky gut<\/a> (intestinal hyperpermeability).<\/li><\/ul>\n\n\n\n<p><strong>Tests for atrophy (nutritional and hormonal deficiencies)<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Vitamins B-1 (thiamine); B-6; B-12; D; E; and folate.<\/li><li>Thyroid: Free T3, Free T4, TSH, reverse T3, thyroid antibodies.<\/li><li>Sex hormones: estradiol, progesterone, testosterone, DHEA, pregnenolone.<\/li><li>Adrenal hormones: AM and PM <a href=\"https:\/\/www.abc.net.au\/news\/2016-10-13\/research-shows-possible-link-between-cortisol-and-alzheimers\/7930728\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">cortisol<\/a>, the stress hormone (via the <a href=\"https:\/\/www.gdx.net\/product\/adrenocortex-stress-hormone-test-saliva\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">Genova Adrenocortex Stress Profile<\/a>).<\/li><li><a href=\"https:\/\/www.drweil.com\/health-wellness\/body-mind-spirit\/heart\/elevated-homocysteine\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">Homocysteine<\/a> (an amino acid linked to brain atrophy, which can be lowered by taking adequate amounts of B vitamins).<\/li><li>Fasting insulin; fasting blood sugar; HbA1c (hemoglobin A1c). All test for <a href=\"https:\/\/www.mayoclinic.org\/diseases-conditions\/alzheimers-disease\/in-depth\/diabetes-and-alzheimers\/art-20046987\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">insulin resistance and early diabetes<\/a>, both common with early dementia.<\/li><li>Serum zinc, copper, <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/24496077\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">glutathione<\/a>, selenium, red blood cell magnesium.<strong>\u00a0<\/strong><\/li><\/ul>\n\n\n\n<p><strong>Tests for toxins (chronic infections, environmental toxins)<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/20847438\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">Mercury<\/a> (inorganic mercury=dental fillings; methyl mercury=fish).<\/li><li>Arsenic, <a href=\"https:\/\/www.sciencemag.org\/news\/2013\/12\/bolstering-link-between-alzheimer-s-disease-and-lead-exposure\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">lead<\/a>, <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/28511080\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">cadmium<\/a>.<\/li><li><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5008232\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">Lyme disease screening<\/a>.<\/li><li><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4789584\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">Mycotoxin<\/a> (black mold sensitivity) screening.<\/li><li><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4789584\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">C4a<\/a> (inflammatory marker for Lyme and mold toxicity).<\/li><li><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4789584\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">TGF-Beta 1<\/a> (inflammatory marker).<\/li><li>HLA-DR\/DQ (genetic test for susceptibility to chronic inflammation of Lyme and mold).<\/li><li><a href=\"https:\/\/newatlas.com\/gut-bacteria-ptsd-alzheimers\/51959\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">Intestinal microbiome<\/a> (gut bacteria).<\/li><\/ul>\n\n\n\n<p><strong>Other useful dementia-related tests<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li><a href=\"https:\/\/www.aarp.org\/health\/brain-health\/info-07-2013\/hearing-loss-linked-to-dementia.html\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">Hearing<\/a> (long-term hearing loss increases risk for dementia).<\/li><li><a href=\"https:\/\/www.medicalnewstoday.com\/articles\/270710.php\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">Cholesterol profile<\/a> (high LDL increases risks for dementia caused by multiple small strokes).<\/li><li><a href=\"https:\/\/www.sciencedaily.com\/releases\/2017\/11\/171110084325.htm\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">Sleep study<\/a> (untreated sleep apnea can cause brain amyloid deposits).<\/li><li>MRI of brain with measurements of brain volume (for people at high risk or who have mild cognitive decline).<\/li><li>Professional screening of your mental status by a psychologist.<\/li><\/ul>\n\n\n\n<p>Will your health insurance pay for all this? In fact, most of the tests listed above are not considered alternative or experimental and indeed are covered by most PPO policies, although with health insurance nothing is for certain.<\/p>\n\n\n\n<p>It\u2019s easier to list tests that are only partially covered: the functional tests for leaky gut and adrenal function. Not covered by insurance are the confirmatory tests for Lyme (<a href=\"https:\/\/igenex.com\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">Igenex<\/a>) and mold (<a href=\"https:\/\/www.greatplainslaboratory.com\/gplmycotox\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">Great Plains Mycotox Profile<\/a>).<\/p>\n\n\n\n<p>If you have concerns about your susceptibility to Alzheimer\u2019s, print this series of Health Tips and take them to your primary care physician.
