It\u2019s Always Been The Carbs."},"content":{"rendered":"\n<p>Somewhere in my library is a copy of Gary Taubes\u2019 2008 <a href=\"https:\/\/www.amazon.com\/Good-Calories-Bad-Controversial-Science\/dp\/1400033462?SubscriptionId=AKIAILSHYYTFIVPWUY6Q&amp;tag=duckduckgo-ffab-20&amp;linkCode=xm2&amp;camp=2025&amp;creative=165953&amp;creativeASIN=1400033462\" target=\"_blank\" rel=\"noreferrer noopener nofollow\"><em>Good Calories, Bad Calories<\/em><\/a>, one of several books that appeared in the wake of the really vicious controversy that surrounded Robert C. Atkins, MD\u2019s <a href=\"https:\/\/www.amazon.com\/Dr-Atkins-Diet-Revolution-Calorie\/dp\/B0006C4BC0\" target=\"_blank\" rel=\"noreferrer noopener nofollow\"><em>Dr Atkins\u2019 Diet Revolution<\/em><\/a>, which first appeared in 1972, has been published in 13 revisions and editions, and continues to sell around the world.<\/p>\n\n\n\n<p>It\u2019s challenging to convey the extent of the controversy regarding Atkins\u2019 suggested way of eating and the animosity that followed. He was flat-out going against what every physician had learned in medical school from the 1940s onward about a healthful diet, weight control, and disease prevention.<\/p>\n\n\n\n<p>Even though Americans were getting fatter by the minute and their cholesterol levels were rising, the treatment Atkins promoted was not eating less fat, but rather eating fewer carbohydrates, especially simple carbs like sugar, starches, and refined flour.<\/p>\n\n\n\n<p>Eat all the meat you want. Eggs and bacon for breakfast? It\u2019s the best breakfast! Eat as much fat as you like. Worst breakfast? Cereal with fruit or a fruit smoothie (<a href=\"https:\/\/www.bhf.org.uk\/informationsupport\/heart-matters-magazine\/nutrition\/5-a-day\/smoothies\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">most of which are loaded with sugar<\/a>).<\/p>\n\n\n\n<p>The seriously overweight US public, dropping like flies from heart attacks, loved what Dr. Atkins was recommending. Millions went low-carb, bought his line of packaged foods and supplements, and supported him to the end. Pasta sales plummeted.<\/p>\n\n\n\n<p>Atkins\u2019 bitterest opponent was Dean Ornish, MD, whose <a href=\"https:\/\/www.amazon.com\/Ornishs-Program-Reversing-Heart-Disease\/dp\/0804110387?SubscriptionId=AKIAILSHYYTFIVPWUY6Q&amp;tag=duckduckgo-ffsb-20&amp;linkCode=xm2&amp;camp=2025&amp;creative=165953&amp;creativeASIN=0804110387\" target=\"_blank\" rel=\"noreferrer noopener nofollow\"><em>Reversing Heart Disease<\/em><\/a> touted extra-low-fat eating, skinless breast of chicken, egg-white omelets, and lots of fruit and veggies. He was, and remains, the quintessence of everything a conventional doctor knows (or thinks she knows) about healthful eating.<\/p>\n\n\n\n<p>Ornish virtually accused Atkins of being a mass murderer with his diet advice. The writing style of <a href=\"https:\/\/en.wikipedia.org\/wiki\/Atkins_diet\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">this Atkins Wikipedia <\/a>entry makes me wonder if Ornish got his way with the editorial content. Want to see a bunch of conventional docs trying to rip Atkins? <a href=\"https:\/\/www.youtube.com\/watch?v=J29eNP0hRyk\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">Check out this video from 2000.<\/a> These guys are mean.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Follow the money<\/strong><\/h3>\n\n\n\n<p>Gary Taubes is an excruciatingly well credentialled science writer with no products except his books to sell anyone. I can picture him sitting in some medical library with a couple of laptops, poring over centuries (yes, centuries) of evidence. Not just clinical studies, but medical anthropology stuff as well. Why do Innuits with a 100% fatty meat\/fish diet and virtually no grains or veggies have zero heart disease? Why did the Atkins program actually seem to work? Why the degree of hostility?<\/p>\n\n\n\n<p>I have to thank a patient who suggested I read Taubes\u2019 2011 book <a href=\"https:\/\/www.amazon.com\/Why-We-Get-Fat-About\/dp\/0307474259\/ref=sr_1_1?keywords=Why+We+Get+Fat+and+What+We+Can+Do+About+It&amp;qid=1580664606&amp;s=books&amp;sr=1-1\" target=\"_blank\" rel=\"noreferrer noopener nofollow\"><em>Why We Get Fat and What To Do About It<\/em><\/a>. Honestly, I wasn\u2019t sure I \u00a0\u00a0was up for yet another diet book. But Taubes\u2019 tome was definitely not a diet book. He wanted to explain the physiology without unrealistic claims. It\u2019s not that complicated if you read with a pencil in hand.<\/p>\n\n\n\n<p>The key point is this: After nearly 50 years since the Atkins book, there\u2019s little doubt that all along Atkins was correct and Ornish, along with the vast conventional medicine community, has been wrong.<\/p>\n\n\n\n<p>Why did this conflict occur? Maybe it\u2019s just the Chicagoan in me, but, folks, the rule in this town is <u>follow the money<\/u>.<\/p>\n\n\n\n<p>Until the 1940s or so, doctors did believe the obesity and heart disease villain was sugar. If you wanted to drop weight and\/or limit your risk of heart disease and other chronic illnesses, just cut back on sugar.<\/p>\n\n\n\n<p>This meant avoiding simple carbs <em>and<\/em> complex carbs. Not only sugar, but all breads, rice, and the like. In other words, avoiding what we now call high-glycemic foods (foods that quickly raise blood sugar).<\/p>\n\n\n\n<p>Then in the 1950s, with heart disease and obesity on the rise, researchers identified the two potential villains as sugars and fats. But the sugar industry funded research with a manipulated outcome that shifted the blame onto fats. <a href=\"https:\/\/www.npr.org\/sections\/thetwo-way\/2016\/09\/13\/493739074\/50-years-ago-sugar-industry-quietly-paid-scientists-to-point-blame-at-fat\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">Here\u2019s the history.<\/a> Read it for a surge of anger.<\/p>\n\n\n\n<p>You can\u2019t be too surprised at a revelation like this. After all, the AMA received millions from the tobacco industry before commenting on the damaging health effects of tobacco. <a href=\"https:\/\/csts.ua.edu\/ama\/cigarettes-damage-the-heart\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">This was <em>ten years after<\/em> the health problems attributed to cigarettes were published in the 1964 Surgeon General\u2019s Report.<\/a> You\u2019ll love this link in which <a href=\"https:\/\/books.google.com\/books?id=lk37egfoiBoC&amp;pg=PA714&amp;dq=Fishbein+Nothing+but+Kent+cigarettes&amp;hl=en&amp;newbks=1&amp;newbks_redir=0&amp;sa=X&amp;ved=2ahUKEwibj_zxtrPnAhUaHc0KHYHgABEQ6AEwAHoECAQQAg#v=onepage&amp;q=Fishbein%20Nothing%20but%20Kent%20cigarettes&amp;f=false\" target=\"_blank\" rel=\"noreferrer noopener\">JAMA editor Morris Fishbein, MD, tells a reporter he keeps nothing but Kent cigarettes on his desk<\/a>. Here\u2019s still more on how <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC2563588\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">Fishbein used his influence to defend the tobacco industry<\/a>.<\/p>\n\n\n\n<p>Although you can order Taubes\u2019 2011 book, my suggestion is to wait a few weeks for his newest title <a href=\"https:\/\/www.amazon.com\/Case-Keto-Rethinking-Practice-Low-Carb\/dp\/0525520066?SubscriptionId=AKIAILSHYYTFIVPWUY6Q&amp;tag=duckduckgo-ffnt-20&amp;linkCode=xm2&amp;camp=2025&amp;creative=165953&amp;creativeASIN=0525520066\" target=\"_blank\" rel=\"noreferrer noopener nofollow\"><em>The Case for Keto: Rethinking Weight Control and the Science and Practice of<\/em> <em>Low-Carb\/High-Fat Eating<\/em><\/a>, which I suspect will include more info on the intermittent fasting he\u2019s alluded to in previous books.<\/p>\n\n\n\n<p>To learn right now about low-carb\/high-fat eating, <a href=\"https:\/\/www.dietdoctor.com\/low-carb\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">click here for the Diet Doctor website,<\/a> whose beginners page is filled with helpful information. <a href=\"https:\/\/www.dietdoctor.com\/is-intermittent-fasting-the-new-norm\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">They cover the science on intermittent fasting nicely too.<\/a><\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Take-home nuggets on carbs<\/strong><\/h3>\n\n\n\n<p>There are several take-aways from Taubes, Diet Doctor, and this Health Tip:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>The scoundrel is the hormone insulin, pumped out by \u00a0your pancreas in response to all forms of sugar. If you have too much insulin, you\u2019ll drive fat into your fat cells and gain weight. Rising insulin triggers weight gain, which triggers more insulin because you develop insulin resistance, which in turn leads to <a href=\"https:\/\/www.mayoclinic.org\/diseases-conditions\/metabolic-syndrome\/symptoms-causes\/syc-20351916\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">metabolic syndrome<\/a>, which increases your risk for heart disease and diabetes.<\/li><li>Insulin levels are driven upward by carbs and by your declining sex hormones. Women often gain weight during <a href=\"https:\/\/wholehealthchicago.com\/blog\/2009\/05\/05\/menopause\" target=\"_blank\" rel=\"noreferrer noopener\">menopause<\/a> because their estrogen levels drop and their bodies store more fat. Many women patients have told me, \u201cI\u2019m eating just the same or even less, and exercising, and I\u2019m still gaining weight.\u201d Repeat after me: It\u2019s the carbs.<\/li><li>Cutting calories, starvation diets, trimming away fat, and eating low-fat products are all useless. They\u2019ll just make you feel miserable and hangry.<\/li><li>Exercise for weight loss is less than useless. Exercise is extraordinarily healthful for many reasons, but as a weight loss aid it will only make you hungry. \u00a0In the memorable words of UK cardiologist Aseem Malhotra, MD: You can\u2019t outrun a bad diet.<\/li><li>Low-carb\/high-fat eating includes meat, poultry, fish, shellfish, and eggs. Eat plenty of greens and other vegetables that grow above ground (broccoli, cauliflower, <a href=\"https:\/\/www.dietdoctor.com\/low-carb\/vegetables\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">click here for more<\/a>). Enjoy healthful fats (butter, olive oil, and coconut oil) and cheeses, avocado, and olives. <a href=\"https:\/\/www.dietdoctor.com\/low-carb\/fats-sauces\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">Here\u2019s a helpful list of fats and sauces and their carb contents.<\/a><\/li><li>Avoid sugar and sugar-containing, starchy foods like beans, potatoes, rice, bread, pasta, \u00a0carrots, parsnips, corn, peas, French fries, and potato chips.<\/li><li><a href=\"https:\/\/www.dietdoctor.com\/low-carb\/20-50-how-much\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">Click here<\/a> for another useful visual guide to carbs from Diet Doctor.<\/li><li>As a bonus, I\u2019ve had more than a few patients tell me eating low-carb\/high-fat vastly improved their IBS, skin conditions, and other ailments.<\/li><\/ul>\n\n\n\n<p>Where do the majority of US physicians stand on all this? A typical primary care physician, if she sees you\u2019re overweight and your cholesterol is high, will start you on statins and have a nutritionist teach you a low-fat diet.<\/p>\n\n\n\n<p>She likely hasn\u2019t read <a href=\"https:\/\/www.scientificamerican.com\/article\/why-almost-everything-dean-ornish-says-about-nutrition-is-wrong\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">this article from Scientific American<\/a> telling the world that Ornish was wrong all along.<\/p>\n\n\n\n<p>Be well,<br><em>David Edelberg, MD<\/em><\/p>\n","protected":false},"excerpt":{"rendered":"<p>Somewhere in my library is a copy of Gary Taubes\u2019 2008 Good Calories, Bad Calories, one of several books that appeared in the wake of the really vicious controversy that surrounded Robert C.
